The Georgia Department of Public Health, Chronic Disease Prevention Section, is pleased to announce a scholarship opportunity to attend a three-day certified diabetes educator (CDE) seminar facilitated by Emory University.   Professionals preparing to take the Certified Diabetes Educator (CDE) examination will find that the seminar facilitates their study.  The program supports public health nurses, pharmacists, health educators, and dieticians. There will be a total of 4 scholarships awarded.

The scholarship will include the cost of registration and lunch. The scholarship will not include travel, meals, parking, or examination fee.

How to Apply

The Scholarship Committee requests a completed application.  All pertinent details regarding the three-day certified diabetes educator (CDE) seminar, eligibility requirements to sit for the CDE examination, and the scholarship application can be found in the attached application. All complete application must be submit to Allison.Smith@dph.ga.gov no later than May 1, 2018 by 5:00pm.

Notification of Award

Applicants will be notified of the status of their application by the Chronic Disease Prevention Section by May 7, 2018.

Scholarship recipients will attend the May 2018 seminar (May 21-23, 2018).
