Georgia OB/GYNs play a critical role in improving maternal outcomes—and now, there’s a powerful resource to help support your highest-risk patients between visits.
The Georgia Department of Public Health (DPH) launched the Perinatal Health Partnership in late 2023 to provide home visiting services for pregnant and postpartum individuals at elevated risk of adverse outcomes.
Backed by recommendations from the state’s Maternal Mortality Review Committee, this program helps close the gap between office visits by offering in-home clinical assessments to monitor maternal health and screen for early warning signs.
This no-cost program is staffed by experienced public health nurses and trained professionals and is designed to serve women facing a range of challenges, including:
- Chronic or mental health conditions
- History of poor pregnancy outcomes
- Pregnancy complications
- Substance use disorder
- Social barriers to accessing care
OB/GYNs are vital partners in the success of this initiative. Provider referrals are essential to connecting eligible patients with services that can make a difference in their health and recovery.
Currently available in 50 counties, Perinatal Health Partnership will expand to 25 more counties this fall.

Perinatal Health Partnership
The Perinatal Health Partnership (PHP) Program provides home visiting services during pregnancy and up to the
infant’s first birthday following delivery. Pregnant women with high-risk conditions or have identified risk factors
that put them at risk for poor pregnancy outcomes, will be targeted for home visiting services and interventions but
enrollment can occur after delivery.
PHP provides additional monitoring between provider appointments that will increase the opportunity to identify
warning signs, complications, and problems earlier and refer to provider for further assessment and intervention
when indicated.
PHP Maternal Home Visiting Service Components:
Clinical assessment for pregnancy and postpartum
complications including:
- BP assessment
- Fingerstick for blood glucose (if indicated)
- Weight
- Urine dipstick
- FHTs
Screening for pregnancy and postpartum warning signs
and symptoms:
- Vaginal bleeding or fluid leaking during pregnancy
- Vaginal bleeding or discharge after pregnancy
- Severe Headaches
- Severe abdominal pain
- Extreme swelling of hands or face
- Changes in vision
- Severe swelling, redness, or pain in leg or arm
- Trouble breathing
- Chest pain or fast-beating heart
- Depression and anxiety
PHP Infant Home Visiting Service Components:
Clinical assessment including:
- Feeding
- Weight
- Head circumference
- Developmental screening
