What is PHP?
Perinatal Health Partnership (PHP)

Provides home visiting services by public health department nurses and trained staff at no cost to patients or providers during pregnancy and postpartum.

PHP home visitors can provide the following monitoring and services for your patients between provider appointments:

  • Blood pressure, pulse, pulse oxygenation
    and weight measurement
  • Fingerstick glucose measurement
  • Urine dipstick
  • Fetal heart tone assessment
  • Review of maternal warning signs &
    symptoms
  • Referral to provider or emergency services
    if indicated
  • Education and linkage to resources
    (e.g., transportation, food, shelter)

 

 


PHP is a state funded initiative targeting pregnant and postpartum moms at higher risk for poor outcomes.

What Providers Should Do​

Partners should refer patients with:

  • Pregnancy related or chronic
    hypertension
  • Preeclampsia
  • Diabetes or Gestational Diabetes
  • Cardiac conditions
  • Pregnancy complications (e.g., preterm
    labor, placenta previa, Lupus, etc.)
  • Substance Use Disorder
  • Mental Health conditions
  • Poor support system
  • Other conditions/circumstances that
    could put them at risk

Success Stories

Overver the last year we have seen:

  • Improved compliance with OB appointments
  • Gestational diabetic compliance with blood glucose monitoring and control
  • Identication of severe blood pressure resulted in emergency referral and admission for preeclampsia
